Job Summary

Serves as a primary point of contact for students in need of support and will facilitate the provision of responsive, culturally sensitive, wrap-around support services for students with immediate and definable needs such as shelter, food, financial, transportation, and other community resource needs.

ESSENTIAL FUNCTIONS

Develop and maintain in-depth knowledge of a diverse and broad range of basic needs programs, services, resources and relevant guidelines to ensure student access to services. Communicate appropriate policies and eligibility criteria to diverse constituencies.

Coordinate care, interventions, social support with appropriate referrals, and follow up services within a broad network of on and off- campus and community resources

Assist in the identification of potential community-based resources that can be a source of support for students.

Conduct individualized, solution-focused assessment, develop recommendations, and serve as centralized point-of-contact to facilitate students' access to campus and community-based resources. Provide follow-up with students to include email reminders, phone calls, and other means of contact to identify program outcomes.

Coordinate case management and support for students in crisis and students in need of support related to food, housing, and financial insecurities in coordination with the Dean of Student Success offices, Counseling and Ability Services providers, Financial Aid, HCC Police Department, Enrollment Management, and Instructional units.

Coordinate closely with multiple campus constituencies to design and implement programming focused on wraparound resource education and outreach.

Develop, coordinate and conduct presentations or workshops related to program services and resources.

Develop marketing materials to disseminate program information, ensure the college website is maintained up to date with relevant program services and resources information.

Attend on/off campus events as the key representative of the basic needs program.

Create and monitor an evaluation process for application tracking, pre- and post- event participation, usage of resources, short and long-term impact, ongoing reporting, etc.

Monitor the program’s budget and make recommendations regarding allocation of resources and expenditures.

Monitor key decisions to ensure alignment to project success criteria throughout the project lifecycle.

Perform other duties, tasks and assignments as required.

QUALIFICATIONS

Education & Experience

         Bachelor’s degree required

         Master’s degree preferred

         3 years experience coordinating wrap around support service programs required

         Experience working in higher education or non-profit organizations preferred

         Experience working with grant-funded training/education programs and reporting preferred         

Licensing & Certification

         Valid Texas Driver License

Working Conditions

General Office.  Must be able to perform all job requirements with or without reasonable accommodations; remain in a stationary position during shift; move items weighing up to 25 pounds; position self to operate job equipment; apply established protocols in a timely manner.  Must access, input and retrieve information from technology devices; communicate with others to accomplish job requirements.  May be required to work after hours to include weekends and holidays.
